Are there any "generic" business forms to get started with? - Personal Trainer Community - Forum

Hello! I am just starting my PT business in my home gym and need all of the business forms ( Par Q, physical assessment, goals, etc) and was wondering if there are any online for a low cost or for free. I am developing my logo etc and will shortly have that to be added to the forms. Or do most PTs now just do everything online through sites like GymPro?

Hi,

I don't know what most PTs do, but I pinched mine from various places on the internet...for free. I re-formatted them, so I could personalize the documents. I don't have a logo right now per say, but I took my cert logo with my name and email address and placed it at the top of the PAR-Q, Medical History, Physician Clearance, and Waiver.

I believe that Christina (the founder of the Forum) and others have posted forms here on the site. You just have to dig around.

Hello again!
Geez! I've searched this site for forms and came across a few but they were too outdated or I wasn't able to open them because of some error. If anyone has a website, company or just can download their forms that would be great! I am looking for all of the start up forms necessary for an in home personal training business such as, Fitness Assessment (including bodyfat,girth measurements etc), Goals, Par Q, Medical History,Nutrition and Health status.
ANY help would be so appreciated!
