    Default Business Forms

    Hey, I am not sure that this has ever been addressed before, but I wanted some feedback on the forms I use and want to know how everyone goes about using them. For example, I have the following forms for consultation:

    -Personal Training Agreement
    -Medical History/Past Exercise Experiences/Goals etc
    -Par-Q
    -Informed Consent for Exercise Testing
    -Waiver

    Side Note: I work in a fitness center as the attendant as well as a trainer, so this is where I sell myself most of the time.

    Now, that is a lot of paperwork to fill out, so my idea was to give it to the potential client and have them bring it back to go over their goals and expectations with them prior to an assessment; I would use the SMART Goals sheet to elaborate on their goals, but obviously following the SMART structure. From there we would go to the assessment.

    SMART Goals: Goals.docx
    Assessment: Assessment.pdf
    Doctor's Consent form (if needed) Doctor's Consent Form.doc

    Sorry if this is very choppy grammatically speaking... Let me know what you think of my forms if I am forgetting anything or if there is a better way to go about doing it. Thanks!

    Brian, I use PAR-Q, waiver, cancellation policy, goals, and a contact info/stats sheet where I write down baseline measurements.

    I looked at your paperwork and it seems pretty much by the book. When I first started training I had my clients fill out all sorts of things. Over time I realized it was not necessary to fill out so much paperwork. Personally I cover 2 things: liability and goals. The other stuff I no longer bother with. The choice is yours though. Over time you will decide what is and is not necessary.

    my only possible red flag might be the agreement form. I dont think agreements are legally enforceable. Maybe change it to "informed consent" but even then its best to have an attorney who lives in your state make that up. the same goes for the waiver
