Default Client with very large hernia in upper abdomen.

I have a client who has a very large hernia in his upper abdomen. It is probably about the size of a grapefruit, maybe bigger. He has had it for years and his doctor did not want to operate. Has anyone else trained someone with a hernia in this region? Any advice?

How old is the client? How long has he had the hernia? What caused it (freak acident, occupatioin, lifting, sneezing, congential abnormaliy, etc)?
Does it interfere with daily activity? Why did the doctor elect not fix?

    - The client is in his mid 60s.
    - He has had it for 10 years.
    - It does not interfere with daily activity. It has been with him for so long he really understands his limitations.
    - He told me why his doctor elected not to operate but honestly I cannot remember. I was so concerned when I saw it that I didn't fully listen to his reason why the doctor did not fix it. I will ask him again next week.

I am not sure how to do core strengthening exercises due to its size. :?:

He also has arthritis really bad in one of his knees (confirmed by his doctor). I had him do a quarter squat and he said it was painful. He suspects arthritis in one of his hips as well. I suggested aquatic exercises (he has a heated pool) and he was very interested.

His upper body strength is fine. No other issues.
