PAR-Q or HHQ?
I personally have never used the PAR-Q, but reviewing my notes from the NSCA CPT book, it says that you may want to use both.
Which form do you use when first meeting with a client?
Do the clients fill out both forms willingly, or reluctantly?
Has any client ever mentioned to you that they seem redundant?

I agree the par Q would be the bear minimum you would want people to complete prior to working with them but I agree a good health history form is better. You could also work the PAR A into your HHQ if you like.
Where the PAR Q would definitely be a good thing is in health clubs. I’ve read that many clubs do not screen members before letting them join. by having potential member fill out the PAR Q first it would help screen out high risk people. Have those people get a note from their doctor before joining.
