Picked up a good book today called Optimal Muscle Training: Biomechanics of lifting for maximum growth and strength by Dr. Ken Kinakin. (It comes with a DVD.) It demonstrates how to perform self tests, functional muscle tests, exercise tests, as well as proper lifting technique and corrective training. It's a couple of years old (2004) so I'm sure some of you already have it. It's published by Human Kinetics. I've learned quite a bit and I've only viewed a few segments so far.
